In this article diagnosis and treatment of a case of endometriosis in an episiotomy scar is presented and discussed in the light of the literature. The patient is a 25-year-old woman suffering from tenderness and swelling in her genital region. On the examination, there was a well healed episiotomy scar on the right and a 1x1.5 cm-diameter apparently cystic lesion with an irregular border at the posterior side of her episiotomy scar. Surgical procedure for the lesion is performed and it is diagnosed as endometriosis histopathologically. An episiotomy scar is a rare site for endometrial implantation. Clinical presentations of this condition are tender perineal nodule, cyclical pain and discharge. Perineal endometriosis is best managed by complete excision.
