A SHOCKING CAUSE OF KIDNEY FAILURE: ANTIPHOSPHOLIPID ANTIBODY SYNDROME

This letter to the editor describes a case of probable Catastrophic Antiphospholipid Syndrome (CAPS) in a 64-year-old male presenting with acute kidney injury, thrombocytopenia, and multi-organ failure. Clinical findings included hypertension, tachycardia, and livedo racemosa, with laboratory results supporting thrombotic microangiopathy. Despite the immediate initiation of "triple therapy" consisting of plasma exchange, high-dose steroids, and anticoagulation, the patient succumbed to widespread thromboembolic events. CAPS is a rare condition with high mortality; early recognition of clinical markers and prompt initiation of aggressive multimodal therapy are crucial to avoid diagnostic pitfalls and improve outcomes in fulminant cases.
