Using intravenous contrast, photon-counting computed tomography angiography (PCCT-A) provides detailed images of intracranial vessels. We report a case in which PCCT-A revealed a 1.5 mm aneurysm at the bifurcation of the middle cerebral artery that was not seen on magnetic resonance angiography. This case highlights the clinical value of PCCT-A in noninvasively detecting small intracranial aneurysms.
