Frontalis suspension is the preferred technique for severe blepharoptosis with poor levator function; however, it requires adequate frontalis muscle activity. In the physical examination of the patient, the patient was diagnosed with severe blepharoptosis. Ipsilateral frontal suspension could not be performed because the patient's ipsilateral frontal muscle was absent, caused by the trauma. The patient underwent cross-frontalis sling to the contralateral frontal muscle with a tensor fascia lata strip. In the postoperative period, satisfactory eyelid lift and symmetry were achieved. During the 24-month follow-up period, no recurrence or complications were observed, and both functional and cosmetic results were positive. Cross-frontalis suspension using the tensor fascia lata may be an effective alternative in selected patients when the ipsilateral frontalis function is absent.
