A 65-year-old male patient presented with exertional chest pain. After evaluation, he was diagnosed with non-ST elevation myocardial infarction. His medical history included type 2 diabetes mellitus, hypertension, and hyperlipidemia. Coronary angiography revealed multivessel coronary artery disease, and an early elective coronary artery bypass grafting (CABG) surgery was planned. During angiographic imaging, an incidental finding of the first septal perforator artery originating from the left main coronary artery (LMCA) was detected.
