Abstract Perivascular spaces, known as Virchow-Robin spaces, are fluid-filled spaces around cerebral vessels. Enlarged forms, termed tumefactive perivascular spaces, can mimic tumors and lead to diagnostic challenges. This case report discusses a 58-year-old female with tinnitus, whose MRI revealed a non-enhancing cystic lesion in the left anterior temporal lobe, typical of anterior temporal lobe perivascular spaces. These lesions can be mistaken for tumors, leading to unnecessary interventions like surgeries or biopsies. Recognizing these lesions accurately is vital to avoid unnecessary treatments. Generally stable over time, these lesions often do not require treatment and can be categorized as "do not touch" lesions.
