AORTIC ANEURYSMS: ENDOVASCULAR AT ALL COSTS?

The endovascular revolution in the last 30 years has completely changed and developed vascular surgery. In the mid-nineties of the last century, first attempts to exclude infrarenal abdominal aortic aneurysms (AAA) by endovascular means (EVAR) started and soon demonstrated the minimal invasive advantages that obviously benefitted many patients. In the following years, we started treating acute and even ruptured infrarenal aneurysms with EVAR under local anesthesia. Since the beginning of this century, we moved up in the aorta, first with fenestrated and later with branched grafts, to treat more complex abdominal and even thoraco-abdominal aneurysms (TAAA) by F/BEVAR. Finally, since a few years, we do have fenestrated and branched grafts to treat many arch pathologies. Specific pathologies such as post-dissection TAAA and aneurysms in connective tissue disease patients are nowadays also regularly addressed with endovascular technologies.
