Background: Artificial intelligence (AI) is increasingly transforming healthcare management by enhancing decision-making, leadership practices, and organizational learning processes. Despite the growing adoption of AI technologies in healthcare organizations, their integration with established management and organization theories particularly transformational leadership and organizational learning remains underexplored. Existing literature has largely focused on clinical applications and technical performance, leaving a gap in understanding how AI reshapes leadership behaviors and facilitates continuous learning at the organizational level. Review text: This narrative review synthesizes the literature on AI-enhanced transformational leadership and organizational learning in healthcare organizations. Drawing on Bass's transformational leadership theory and Argyris and Schön's organizational learning framework, the review examines how AI technologies, including predictive analytics, machine learning, natural language processing, and clinical decision support systems, support the four dimensions of transformational leadership: idealized influence, inspirational motivation, intellectual stimulation, and individualized consideration. It further explores how AI accelerates organizational learning through single-loop and double-loop processes by enabling rapid knowledge generation, pattern detection, knowledge dissemination, and evidence-based decision-making. A conceptual model is proposed to illustrate these mechanisms, and case examples from IBM Watson Health, Johns Hopkins Hospital, and Google Health are discussed to demonstrate AI's role in improving clinical decision support, operational efficiency, patient flow, and organizational knowledge management. Contextual factors such as ethical AI governance, organizational culture, AI literacy, data quality, and change management are also considered as moderators of these relationships. Conclusion: AI has significant potential to strengthen transformational leadership and organizational learning in healthcare organizations by enabling data-driven innovation, personalized leadership approaches, scalable knowledge sharing, and adaptive capacity. However, sustainable benefits depend on complementary investments in leadership development, workforce readiness, ethical governance, and organizational culture rather than technology alone. This review contributes to the healthcare management literature by linking AI applications with established management theories and provides directions for future research on the long-term organizational impacts of AI across diverse healthcare settings and resource contexts.
