Diffuse tenosynovial giant cell tumor (D-TGCT), formerly known as the diffuse pigmented villonodular synovitis (PVNS) of the shoulder, is a rare condition. This case report presents the diagnosis and treatment of diffuse TGCT in the shoulder of a 20-year-old female patient. The patient underwent arthroscopic synovectomy followed by adjuvant intra-articular Rhenium-186 radiosynovectomy. At one-year follow-up, she exhibited full recovery with no clinical or radiological evidence of recurrence. This case highlights the potential effectiveness of combining surgical excision and adjuvant radiosynovectomy to reduce the risk of recurrence in diffuse shoulder TGCT.
