Osteochondroma and osteoma are rare benign tumors affecting the mandibular condyle, often leading to progressive facial asymmetry, malocclusion, and temporomandibular joint (TMJ) dysfunction. Differentiating between these entities is critical for accurate diagnosis and effective treatment planning. This case report presents two patients with unilateral condylar overgrowth but differing histopathological diagnoses. Both patients were treated with a 'customizable condylectomy' approach, which involved the surgical excision of tumor masses tailored to the specific dimensions of each case and the correction of malocclusion and facial asymmetry without the need for orthognathic surgery. By the end of the first year, both patients had stable occlusion, with resolution of symptoms such as pain, bone discomfort, and difficulty eating. This report highlights the effectiveness of the customizable condylectomy approach in treating condylar tumors by simultaneously addressing the underlying pathology and correcting associated facial asymmetry, without the need for additional orthognathic surgery. The approach offers both aesthetic and functional benefits, supporting its value as a definitive treatment option in selected cases.
