BACKGROUND: To report the first documented case of complete resolution of refractory dilatory Eustachian tube dysfunction (ETD) following dupilumab treatment in patients with severe chronic rhinosinusitis with nasal polyposis (CRSwNP). METHODS: Four consecutive patients with longstanding refractory dilatory ETD, severe CRSwNP, and asthma, unresponsive to multiple sinus and mastoid surgeries, were treated with dupilumab. RESULTS: Dupilumab resulted in complete remission of both CRSwNP and dilatory ETD symptoms within 6 weeks. Audiometry and tubomanometry confirmed restoration of middle ear function. CONCLUSION: Dupilumab may offer a novel therapeutic option for patients with dilatory ETD associated with type 2 inflammation. Further prospective studies are needed to assess its long-term efficacy.
