Epstein-Barr virus (EBV) is a common herpesvirus, often presenting as infectious mononucleosis. While mild hepatic enzyme elevations are common in EBV infection, cholestatic hepatitis with significant hyperbilirubinemia is rare, occurring in less than 5% of cases. A 21-year-old female presented with epigastric pain and elevated liver enzymes with cholestatic pattern. Imaging excluded biliary obstruction. Serologic testing confirmed acute EBV infection. Other viral and autoimmune causes were ruled out. The patient improved with supportive care and was discharged. Although uncommon, EBV should be considered in the differential diagnosis of cholestatic hepatitis, particularly in young adults. Timely recognition may prevent unnecessary invasive procedures and ensure appropriate management.
