EFFECTS OF TIRZEPATIDE THERAPY ON ARGININE VASOPRESSIN DEFICIENCY: A CASE WITH REDUCED DESMOPRESSIN REQUIREMENT

Arginine vasopressin deficiency (AVP-D) is a rare endocrine disorder that causes polyuria and polydipsia. The case of a 45-year-old woman with AVP-D who showed a marked reduction in urine output and desmopressin requirement after starting tirzepatide for weight management is presented. Two weeks after she began therapy, the desmopressin dose was gradually lowered because she developed hyponatremia and her thirst improved. By the end of the first month, her desmopressin requirement was less than half the original dose. She also experienced significant weight loss, which matches the expected effects of tirzepatide. Experimental and clinical evidence suggests that GLP-1/GIP receptor agonists may control vasopressin release and water-sodium balance through both hypothalamic and renal mechanisms. This case is the first reported instance of reduced desmopressin requirement in AVP-D associated with tirzepatide therapy. These findings highlight the potential for incretin therapy to modulate water balance and stress the need for further research on safety and physiology in this context.
