EFFICACY OF THE ADDITIONAL EFFECT OF STELLATE GANGLION BLOCK IN COMBINATION OF INTRATYMPANIC STEROID AND HYPERBARIC OXYGEN THERAPY FOR REFRACTORY SUDDEN SENSORINEURAL HEARING LOSS

BACKGROUND: Refractory sudden sensorineural hearing loss (RSSNHL) underscores the need for more effective salvage therapies. This study aimed to investigate the efficacy of stellate ganglion block (SGB) combined with intratympanic steroid (ITS) and hyperbaric oxygen therapy (HBO) as triple salvage therapy in patients with RSSNHL. METHODS: This retrospective study analyzed 73 patients with unilateral RSSNHL admitted between January 2022 and December 2023. All patients underwent salvage therapy with HBO and ITS, with (n = 31) or without SGB (n = 42). The primary efficacy endpoint was the change in pure tone average thresholds, with secondary endpoints comprising demographic characteristics, tinnitus evaluation, and psychological distress scores. RESULTS: Both groups showed post-treatment hearing improvement (P < .05), but the SGB group achieved greater hearing gain, higher efficacy rates, and superior low-frequency recovery (P < .05). Tinnitus relief and psychological outcomes were also significantly better in the SGB group (P < .05). Patients with both shorter intervals from symptom onset to salvage therapy and relatively lower baseline hearing thresholds demonstrated significantly greater hearing improvement. Neither group experienced severe adverse events. CONCLUSION: Lower baseline hearing thresholds and earlier therapy initiation predicted better outcomes. The triple salvage therapy significantly improved hearing recovery in RSSNHL patients, particularly in low-frequency ranges, while also reducing tinnitus severity and enhancing psychological well-being.
