Aims: Mean platelet volume (MPV), a commonly accessible hematological metric, has been suggested as an indicator of platelet activation and thrombotic risk. Nonetheless, information concerning its importance in hereditary thrombophilia remains limited. Methods: This retrospective cross-sectional study comprised 208 patients with thrombophilia and 82 healthy controls. Clinical and laboratory data were obtained from medical records. MPV values were compared across groups. Results: MPV levels were significantly higher in patients with thrombophilia than in healthy controls (7.66+/-1.52 fL vs. 7.25+/-1.41 fL, p=0.028). MPV showed a significant negative correlation with platelet count (r=-0.327, p=0.008), and the receiver operating characteristic analysis yielded an area under the curve of 0.682. Results indicated a modest diagnostic performance of MPV in distinguishing patients with thrombophilia from healthy controls. Conclusions: Patients with thrombophilia had higher MPV values than healthy controls. Although MPV alone showed limited diagnostic utility, the observed findings suggest a potential association between inherited thrombophilia and platelet-related alterations. Further prospective studies are necessary to ascertain the clinical significance of MPV in assessing thrombotic risk.
