Objective: Low-grade fibromyxoid sarcoma (LGFMS) is a rare soft tissue neoplasm characterized by a deceptively benign histopathological appearance but malignant clinical behavior. Case : We report a case of a 50 -year-old female presenting with a localized mass in the distal right humerus, initially misdiagnosed as a schwannoma following biopsy. Contrast -enhanced magnetic resonance imaging (MRI) revealed a well -defined lesion between muscle planes. Definitive surgical excision and subsequent histopathological analysis confirmed LGFMS, with immunohistochemistry demonstrating diffuse MUC4 positivity. Margin expansion surgery was performed, and no recurrence or metastasis was observed during a 24-month follow -up period. Conclusion: The uncommon anatomical location and initial diagnostic error underscore the clinical significance of this case, emphasizing the importance of accurate histopathological and immunohistochemical evaluation.
