Struma ovarii is uncommon (2.7% of ovarian teratomas) and often associated with mature cystic teratomas. Malignant transformation of this tumor is rare (5% to 10%). In this study, a 61 year old female with papillary carcinoma arising from struma ovarii is presented. There was no evidence of clinical malignancy or metastases preoperatively and it was diagnosed as malignant struma ovarii histopathologically after operation. Any adjuvant treatment was not performed after surgical excision because of tumor had no capsular and lymphovascular invasion and dissemination.
