Normal-pressure-hydrocephalus (NPH) may present with psychiatric disorders besides the classical clinic symptoms consisting of dementia, imbalance and incontinence. In this article, we aimed to present a case diagnosed with late onset bipolar disorder and concomitant clinical signs of normal pressure hydrocephalus. 49-year-old male patient admitted to our clinic with complaints of insomnia, irritability, aggression and distractibility for the last 3 months. He was in a state of mania with the following symptoms for the last 10 days; being more talkative, pressure to keep talking, fight of ideas, restlessness. In addition, he complained of amnesia, imbalance, urinary incontinence that started concurrent with symptoms for the last 3 months.The patient was diagnosed with bipolar disorder manic episode according to DSM-5 . It was deemed appropriate for the patient to be hospitalized. Blood tests were normal. MRI result was consistent with NPH. In late-onset psychiatric disorders, the diagnosis should be considered as a secondary presentation due to other medical conditions should be investigated.
