Background: Although various techniques have been described for the treatment of circular skin defects in the face, new techniques are needed. In this study, the author’s experience with a new local flap technique named Modified Reading Man Flap (Modified RMF) is presented for the reconstruction of circular skin defects. Materials and Methods: Eighteen patients’ (12 women and 6 men) circular skin face defects were treated with Modified RMF between June 2015 and October 2023. The patients were ranged from 22 to 83 years and the mean age of the patients was 60. Technique was used after the excision of basal cell carcinoma (BCC) in 14 patients, congenital nevus in four patients. Results: A tension-free closure was achieved in all patients. Hematoma, infection, and dehiscence were not seen in the postoperative period. Only, superficial distal flap (f2) necrosis was seen in one patient. Satisfactory cosmetic results were achieved in all patients. Conclusion: The modified RMF technique may be a viable alternative choice for tension-free closure of circular skin defects in the face, following BCC and congenital nevus excision.
