A 32-year-old male presented with a penile hematoma after using a penile chastity cage for sexual gratification. Physical examination revealed distal penile edema and diffuse hematoma without urethral hemorrhage or incontinence. Doppler ultrasonography showed no cavernosal or fascial defect but demonstrated a subcutaneous hematoma. Management included ice application, Coban compression bandage, analgesic-anti-inflammatory treatment, and oral antibiotic prophylaxis. The bandage was changed daily; symptoms improved by 48 h, it was removed at 1 week, and complete resolution occurred by week 3. At 6 months, sexual function was normal. This case highlights potential risks of such devices and the successful conservative management of a penile hematoma.
