Neonatal appendicitis is a rare but serious surgical condition that often presents with non-specific symptoms and a high risk of perforation. Early recognition is difficult but critical for the survival of the patient. We report a premature male neonate with a history of perinatal asphyxia who developed sudden abdominal distension and bilious vomiting during hospitalization and had surgery afterwards. The patient recovered postoperatively and was discharged on the 35th day of hospitalization. In this case report, we emphasize the importance of suspicion for newborn appendicitis in patients with a history of perinatal asphyxia and prematurity who develop sudden abdominal rigidity and vomiting.
