A case of peritoneal melanosis with left ovarian dermoid cyst is described. Histology showed that the dermoid cyst contained gastrointestinal mucosa associated with focal ulceration and hemorrhagic foci. The areas of pigmentation within the dermoid and the omentum were due to collections of heavily brown-black pigment-laden macrophages. The pigment had the histochemical features of both melanin and hemosiderin. Ultrastructural examination showed premelanosome-like structure, free or membrane-bound and, hemosiderin-like crystalloid material, intrasytoplasmic or extracellular, in the macrophages.
