Objective: To investigate levels of selenoprotein P (SeP), a liver-derived hepatokine, in patients with newly diagnosed type 2 diabetes mellitus (T2DM) compared with healthy controls and to evaluate its relationship with metabolic indicators. Methods: Thirty-nine newly diagnosed T2DM patients and 41 healthy controls were enrolled between November 2024 and April 2025. Demographic and biochemical data were collected, and statistical analyses were conducted to assess the associations between SeP and glycemic and metabolic parameters. Results: Selenoprotein P levels were significantly higher in the diabetic group (3.40 µg/mL) than in controls (2.79 µg/mL) with a p-value of .041. SeP showed positive associations with BMI (p = .021) and triglycerides (p = .008). There was a significant positive correlation between SeP and the TyG index in the whole group (Spearman r = .370, p < .001) and in the T2DM group (r = .395, p = .014). The diagnostic ability (AUC) of SeP was 0.633 (p = .041) with a cut-off of 2.861, showing 76.9% sensitivity and 53.7% specificity. Conclusion: Serum selenoprotein P levels were elevated in patients with newly diagnosed type 2 diabetes mellitus and were associated with body mass index and triglyceride levels. Further studies are needed to clarify the clinical significance of these findings.
