Aims: To describe maternal and neonatal outcomes of pregnancies complicated by Behçet's disease (BD) based on a pooled analysis of published case reports over the past 30 years. Methods: A systematic search of PubMed, Web of Science Core Collection, and Google Scholar was performed to identify case reports and case series of BD in pregnancy published between January 1995 and March 2025. Data on maternal age, disease duration, disease activity, delivery outcomes, and neonatal birth weight were extracted. Results: A total of 61 cases were identified. The mean maternal age was 32.3+/-6.3 years, and the mean disease duration was 5.2+/-3.2 years. Flares during pregnancy were reported in 32.8% of women. Preterm delivery (11.5%) and low birth weight (11.5%) were the most common complications, while miscarriage (3.3%) and preeclampsia (1.6%) were less frequent. The mean neonatal birth weight was 3045+/-517 g. Conclusion: In this pooled analysis, preterm delivery and low birth weight were the most frequent complications among pregnancies with BD. Obstetricians should be aware that BD may first present during pregnancy and should closely monitor affected women, especially in high-prevalence regions.
