To present a rare case of a ruptured pulmonary hydatid cyst initially mimicking pneumonia in the emergency department. A 22-year-old woman was evaluated for acute cough, chest pain, and malaise. Chest radiography and computed tomography revealed a ruptured hydatid cyst in the right upper lung and an additional small hepatic cyst. Surgical consultation was obtained, and operative management was planned based on the imaging findings. The patient underwent successful surgical intervention and recovered fully without postoperative complications. She remained asymptomatic during follow-up. Ruptured pulmonary hydatid cysts can resemble common respiratory conditions, leading to diagnostic delays. This case highlights the importance of considering hydatid disease in patients from endemic regions who present with nonspecific respiratory symptoms and emphasizes the role of timely imaging and surgical management in achieving favorable outcomes.
