Sagliker syndrome is a rare but severe complication of uncontrolled secondary hyperparathyroidism in patients with chronic kidney disease (CKD), most often occurring in individuals receiving long-term dialysis. First described by Yahya Sagliker, the syndrome represents an extreme form of renal osteodystrophy characterized by progressive craniofacial deformity, skeletal destruction, and significant functional impairment. Although secondary hyperparathyroidism is common in end-stage kidney disease, Sagliker syndrome develops only after prolonged exposure to markedly elevated parathyroid hormone (PTH) levels. A case of severe Sagliker syndrome is presented, with a description of its imaging findings.
