Hypertriglyceridemia is a rare, but important complication seen in children with diabetic ketoacidosis (DKA). It may present along with new-onset DKA, thereby adding to the clinical complexity. Very high triglyceride levels can lead to serious complications such as acute pancreatitis, lipemia retinalis, and increased risk of thromboembolic events. Early recognition and prompt action are essential to reverse these metabolic complications. In this case report, we describe a pediatric patient with a new onset of DKA, whose clinical course was complicated by severe hypertriglyceridemia.
