Spontaneous ovarian hyperstimulation syndrome (OHSS) is extremely rare, and almost always related to ovulation induction. Misdiagnosis of OHSS may lead to life threatining intervention. To avoid unnecessary laparotomy and termination of pregnancy, we emphasize the differential diagnosis of OHSS from ovarian cystic neoplasms. We managed two cases of severe spontaneous OHSS conservatively without complication. Postnatal follow-up between 18-24 months is normal in both cases.
