Juvenile fibroadenoma (FA) is the most common benign breast tumor in adolescents representing 80% of benign solid breast masses. FAs are hormonally influenced tumors, usually appearing during childhood and peaking during menarche. Giant juvenile FA (GJFA) is defined as a tumor exceeding 5 cm in diameter, weighing more than 500 g, or affecting over 80% of breast tissue. We report a 13-year-old Hispanic female case with a GJFA measuring 9.5 cm × 4.8 cm × 11.3 cm, weighing 469 g, and occupying 80% of the right breast. Breast reconstruction was successfully performed using a central mound pedicled flap achieving breast symmetry. This case highlights the importance of early diagnosis and individualized management. Written patient consent and ethical approval were obtained, and the report was structured based on SCARE criteria.
