Objective: This study aimed to evaluate the effectiveness of the cartilaginous push-down dorsal preservation rhinoplasty technique in correcting postoperative nasal axis deviation in patients with I- and C-shaped crooked-nose deformities. Methods: Forty-eight female patients undergoing closed cartilaginous push-down rhinoplasty were included. Preoperative and 12-month postoperative nasal deviation angles were measured using standardized digital image analysis and compared statistically. Results: Significant improvement was obtained with the applied method. In I-shaped deformities, deviation improved from 6.5 +/- 2.4 derece to 0.1 +/- 0.3 derece (p < 0.001). In C-shaped deformities, deviation improved from 167.1 +/- 5.3 derece to 179.4 +/- 0.7 derece (p < 0.001). Conclusion: The cartilaginous push-down dorsal preservation technique is effective in correcting I- and C-shaped crooked nose deformities while preserving dorsal continuity and structural integrity.
