Upadacitinib, a selective oral Janus kinase 1 (JAK1) inhibitor, is approved for the treatment of moderate-to-severe Crohn's disease (CD). Sepsis and opportunistic infections associated with upadacitinib have been reported in patients with CD. This report presents the first documented fatal case of sepsis linked to a JAK1 inhibitor, underscoring its potential risk.
