This editorial addresses the growing overlap between metabolic dysfunction-associated steatotic liver disease (MASLD) and chronic viral hepatitis, particularly HBV. It argues that virological quiescence does not equate to biological inactivity in metabolically injured livers, challenging existing disease constructs and clinical algorithms. The article emphasizes the need for a dual-etiology paradigm in hepatology, integrating metabolic risk and fibrosis burden into routine decision-making, and calls for updated guidelines and prospective studies to address this complex interaction.
